At Family Tree, we begin the admission process by meeting with you and your loved one to assess their needs. If we are able to provide the services your loved one needs and their is availability in one of our homes, you will initially be asked to complete an Admission Application and a Financial Information form.
If space is not currently available in one of our homes, you will have the option of completing a Preferred Waiting List Agreement. Once this form, along with a $250 security deposit is received, you will be placed on our waiting list. When space becomes available, the security deposit will be applied to the first months rent upon admission. If you choose to be removed from the waiting list, the security deposit will be refunded.
Admission criteria:
• We accept individuals 55 and older
• Residents may ambulate with a cane or walker
• Residents must be able to dress themselves, and do so with minimal assistance
• Residents must not wander
• We can manage insulin dependent diabetics
• Residents must be continent or semi-continent of bladder and continent of bowel
